The service is to provide comprehensive assessment, brief intervention, sign-posting and onward referral for patients over 18 presenting with mental health complaints.
Ideally, patients presenting with mental health problems should be referred to the mental health practitioner before seeing a GP or at least before a prescription is issued. However those on current medication will also be seen.
Patient who are referred into the service will have an initial comprehensive assessment of 45 minutes. Dependant on need, they can expect to be referred on or signposted to appropriate intervention. (Social Prescribing, talking therapy (IAPT), living well team, community mental health, home based treatment team, psychology services, eating disorders etc) Some patients may benefit from further review by the mental health practitioner, but it is not anticipated that he will hold a long-term case load.
