Clinical Pharmacist Team

Working closely with both the administration team and clinicians, our Clinical Pharmacist team is responsible for helping patients to manage long-term conditions, handles hospital discharge summaries, supports those on multiple medicines, carries out structured medication reviews and enables better access to health checks.
The role is pivotal to improving the quality of care and ensuring patient safety. Having Clinical Pharmacists in GP practices also helps GPs manage the demands on their time, for example, diagnosing and treating patients with more complex conditions.
