New Patients

 St Johns Medical Centre is accepting new patients who live within the practice boundary; to check whether your address falls within the boundary, click here. To register with the practice you will need to complete the following registration form GMS1 Form and new patient application form  New Patient Registration Form . Alternatively please call into the surgery to collect a new patient registration pack.

New temporary and permanent patients to the practice will need to provide identification and proof of address as part of the registration process. A current passport or photo driving license will be accepted for identification and a recent utility bill (within the last 3 months) for proof of address. For children, a birth certificate will be accepted where a passport is unavailable.

New permanent patients can have a simple health screening appointment with our Health Care Assistant to help us to establish accurate medical records, including current medication, allergies, up-to-date vaccinations etc.

St Johns operates as a group practice which means that patients have the flexibility to see any of the eight GP partners for routine appointments based on next available appointment time throughout the practice. In addition we offer emergency same day appointments with the duty doctor and/or nurse practitioner to provide optimum access.
